In order to use the ECG and blood pressure monitoring features in unsupported vicinities or with a non-Samsung phone, you’ll need to install the modded Samsung Health Monitor app from XDA Senior Member Dante63. This app must be installed on both your phone and the Galaxy Watch 4 in order to use these features.

Installing the modded Samsung Health Monitor app on your phone

To install the modded Samsung Health Monitor app on your phone, simply locate the modded app in the Drive folder (labeled Phone.WearOS.SHM.MOD.X.X.X.XXX.dante63.apk) from this link and transfer it to your phone’s root directory.

Next, use a file explorer to navigate to the root directory and locate the modded APK on your phone.

To install it, tap on the modded Samsung Health Monitor APK. You will be prompted to choose whether you want to install apps from unknown sources. Allow by following the prompts and heading over to the device settings.

Another pop-up with an Install button will appear. Tap on it to install the app and then select Done to complete the installation.

Installing the modded Samsung Health Monitor app on the Galaxy Watch 4 requires ADB.

  • Open the Watch SHM MOD WearOS folder in the Google Drive folder from the above link.
  • Now, open the READ ME -ADB Manual instructions folder and extract the contents of the ZIP file in the folder to your desktop.
  • Double-click on the Minimal ADB Fastboot executable from the ZIP file and set up ADB by following the instructions provided in the installation wizard.
  • You will be greeted with a command prompt as soon as the installation is complete. If not, navigate to the directory where Minimal ADB Fastboot has been installed, and double-click on the file labeled cmd-here.

Before continuing, please enable the ADB debugging and Debug over Wi-Fi settings on your Watch. Developer Options must be turned on to do so. To turn on Developer Options, go to the device settings on your watch, scroll down to the About watch option, and select it.

On the next page, select the Software option and then tap on the Software version several times to enable the Developer mode.

  • Now go back to the device settings and tap on Developer options. 
  • Toggle on the ADB debugging and Debug over Wi-Fi settings to enable them.
  • As soon as you are connected to Wi-Fi, tap on the active connection and take note of the IP address stated on the next page.
  • Navigate to the Minimal ADB and Fastboot command prompt window on your PC and enter the following words “adb connect” followed by the IP address from the previous step.
  • You would be prompted to allow debugging. Select OK if you just want to allow debugging one time, or Always allow from this computer if you will not like to repeat the process again.

Type the following ADB command to install the modded Samsung Health Monitor app on your watch. Make sure that the modded Samsung Health Monitor APK is in the same directory as the file labeled cmd-here, and that you replace the X in the command with the current version number of the APK.

adb -s "IP address" install Watch.SHM.MOD.X.X.X.XXX.dante63.apk

A “Success” message will be displayed upon successful installation. Now, enter the below command to disconnect from ADB.

adb disconnect

You can now find the modded Samsung Health Monitor app in your watch’s app drawer. Follow the instructions in the app to connect your Galaxy Watch 4 and set up the ECG and blood pressure features. Once that’s done, add the ECG and blood pressure tiles to your watch. Remember to disable ADB debugging on your Galaxy Watch 4 when you’re not using it, or it will impact battery life.
